Cancellation Policy
1. Consent to Services By signing, you authorize PMLD to perform services such as blood draws, urine collection, swabs, mobile lab tests, and other authorized procedures. Consent & Policy forms must be completed 48 hours prior to service. 2. Appointment Scheduling Appointments must be booked at least 48 hours in advance. Same-day or emergency appointments incur a $50 fee. 3. Cancellation Policy Cancel/reschedule at least 48 hours in advance. No-shows forfeit the deposit. 4. Payment Policy A $100 deposit is due at booking. Full payment is due at the service unless prior arrangements are made. Accepted methods: • Cash (for remaining balance only) • Credit/Debit Card • Zelle/CashApp (for remaining balance only) 5. Service Fees & Travel Includes collection, materials, and local travel. Additional charges: • $0.45/mile beyond 50 miles • Difficult draws • After-hours/weekends: $50 6. Confidentiality We follow HIPAA guidelines. Information is never shared without written consent. 7. Results Delivery Results are sent to the provider or email (for DNA/gender tests). We do not interpret results. 8. Turnaround Time Most results in 3–5 business days. Expedited (1–2 days) costs $175. 9. Specimen Handling We handle collection, labeling, and delivery, but are not liable for lab errors or delays. 10. Client Prep Follow all instructions (e.g., fasting) or samples may be invalid. 11. Minors Parental/legal guardian for consent and presence required for minors. 12. Safety Sterile, single-use equipment is used. Clients must provide a safe, clean space. 13. Right to Refuse We may refuse service if the environment is unsafe, or if the client is aggressive or under influence.
